ABSTRACT

La anemia afecta a miles de niños. Para el diagnóstico se cuantifica la hemoglobina (Hb); técnica que no se encuentra al alcance de toda la población. Contar con un instrumento validado de fácil aplicación, puede ayudar al diagnóstico. El objetivo de este estudio fue validar una aplicación móvil (APP) para diagnosticar anemia en niños de 2 a 5 años, aplicable por padres o tutores. Estudio de validación de escalas. Mediante búsqueda bibliografía se recopilaron ítems y dominios relacionados con anemia en niños. Una vez reducidos, se construyó un cuestionario para pilotaje, con tres hematólogos pediatras. El resultado de este fue posteriormente validado por 22 expertos mediante aplicación de escalas tipo Likert. Los ítems validados, se contrastaron con la Hb de niños de 267 niños de 2 a 5 años de los andes ecuatorianos (2.560 msnm). Se determinó asociación de los ítems con Hb y con los resultados obtenidos y se construyó la APP. 14 ítems fueron analizados. Todos ellos puntuaron sobre la mediana de la distribución (35,5 puntos) y fueron valorados por, al menos el 50 % de los expertos. Se seleccionaron palidez palmar, astenia y sueño en horas no habituales. Todos mostraron asociación significativa con anemia (p<0,05), y fueron aplicados como preguntas a padres o tutores y contrastados con el valor de Hb. Sensibilidad y especificidad para palidez fue: 85,1 % y 85,0 %; astenia: 72,3 % y 87,7 %; sueño en horas no habituales: 68,1 % y 87,7 %; palidez más astenia o sueño: 95,7 % y 74,6 %; y debilidad más sueño: 92,5 % y 76,8 %. Se desarrolló y validó una APP para diagnóstico de anemia en niños de 2 a 5 años aplicable por padres o tutores.


SUMMARY: Anemia affects thousands of children, and to reach a diagnosis, hemoglobin (Hb) is quantified. This technique however, is not always accessible to the general population. Therefore, the availability of a validated instrument can be useful in the diagnosis. The aim of this study was to validate a mobile application (APP), to diagnose anemia in children from 2 to 5 years old, applicable by parents or guardians. Scale validation study. Through a bibliographic search, items and domains related to anemia in children were collected. Once reduced, a pilot questionnaire was constructed with three pediatric hematologists. The result was later validated by 22 experts through the application of the Likert-type scales. The validated items were contrasted with the Hb of children of 267 children from 2 to 5 years of age from the Ecuadorian Andes (2,560 meters above sea level). The association of the items with Hb and with the results obtained was determined, and the APP was constructed. 14 items were analyzed. All of them scored above the median of the distribution (35.5 points) and were valued by at least 50 % of the experts. Palmar pallor, asthenia, and sleep at unusual hours were selected. All showed a significant association with anemia (p<0.05) and were applied as questions to parents or guardians and contrasted with the Hb value. Sensitivity and specificity for pallor was: 85.1 % and 85.0 %; asthenia: 72.3 % and 87.7 %; sleep at unusual hours: 68.1 % and 87.7 %; paleness plus fatigue or sleepiness: 95.7 % and 74.6 %; and weakness plus sleep: 92.5 % and 76.8 %. An APP for the diagnosis of anemia in children from 2 to 5 years old applicable by parents or guardians was developed and validated.

ABSTRACT

Anemia is the most common blood disorder in the world, affecting millions of people yearly. It has multiple causes and jeopardizes development, growth and learning. Current tools provide non-hematologist doctors with just numbers. The present paper proposes a web application intended to provide doctors at Celia Sánchez Manduley Hospital in Manzanillo with a tool for determining the morphologic type of anemia, creating a list of possible causes, and also storing patient data in a database for future researches. As an information system, the website constitutes a powerful tool for the decision-making process, in particular, the diagnostic process, intended to provide more detailed information on anemia as well as to foster the information management at the hospital. For the application, HTML, CSS, JavaScript and PHP were used as languages; Apache as a web server; CodeIgniter as a PHP framework; MariaDB as a database management system; and Visual Studio Code as a development environment(AU)


La anemia afecta a millones de personas anualmente, esta obedece a múltiples causas y compromete el crecimiento, desarrollo y aprendizaje. Las herramientas actuales ofrecen al médico no especialista en hematología solo cifras. El presente trabajo propone una aplicación web con la cual se pretende brindar a los médicos del Hospital Celia Sánchez Manduley en Manzanillo una herramienta para la determinación del tipo morfológico de anemia, la elaboración de un listado de las posibles causas que la originan, así como el almacenamiento de datos de los pacientes para futuras investigaciones, y con ello contribuir a la toma de decisiones, en particular, al proceso de diagnóstico, y a una mejor gestión de información en el hospital. Para la aplicación se utilizaron los lenguajes HTML, CSS, JavaScript y PHP, el servidor web Apache, el framework PHP CodeIgniter, el gestor de base de datos MariaDB y el entorno de desarrollo Visual Studio Code(AU)

Es de conocimiento común que la desnutrición afecta preferentemente a países en vías de desarrollo, donde constituye una causa muy frecuente de morbi-mortalidad en niños y adultos, con predilección en las poblaciones más desfavorecidas. El siguiente estudio descriptivo se realizó en la comunidad indígena Hiwi de San Juan De Manapiare en el Amazonas venezolano con el objetivo de establecer el tipo de parasitismo intestinal más frecuente en dicha población y su relación muy estrecha con la desnutrición y diferentes grados anemia. Se estudiaron las muestras de heces recién emitidas y se examinaron en el microscopio, asimismo se le realizo el examen físico a los participantes del estudio y se utilizaron los datos antropométricos obtenidos para hacer la pertinente comparación con diferentes métodos de referencia para estimar el grado de desnutrición de cada individuo, de igual modo se extrajeron las muestras de sangre para determinar el nivel de hematocrito, hemoglobina, la formula total y diferencial de los leucocitos. Se obtuvo como resultado principal que los helmintos son los parásitos más frecuentes en la población estudiada. El presente trabajo pretende dar una imagen real de las patologías que más afectan a las comunidades originarias en el contexto actual de los derechos indígenas con el objetivo de avivar el interés por las investigaciones en dicha población y de aportar ideas para realizar planes concretos a largo plazo para tratar de mitigar los efectos tan deletéreos que tienen en la salud las enfermedades infecciosas(AU)


It's common knowledge that malnutrition preferentially affects developing countries, where it constitutes a very frequent cause of morbidity and mortality in children and adults, with a predilection in the most disadvantaged populations. The following descriptive study was carried out in the Hiwi community of San Juan De Manapiare in the Venezuelan Amazon with the objective of establishing the most frequent type of intestinal parasitism in said population and its very close relationship with malnutrition and different degrees of anemia. Freshly passed stool samples were studied and examined under the microscope, likewise a physical examination was performed on the study participants and the anthropometric data obtained were used to make the pertinent comparison with different reference methods to estimate the degree of malnutrition of each individual, in the same way, blood samples were extracted to determine the level of hematocrit, hemoglobin, the total and differential formula of leukocytes. The main result was that helminths are the most frequent parasites in the studied population. The present work tries to give a real image of the pathologies that most affect the original communities in the current context of the rights of the indigenous with the aim of stimulating interest in research in this population and contributing ideas to carry out concrete long-term plans period to try to mitigate the deleterious effects that infectious diseases have on health(AU)

ABSTRACT

Anemia is the most common extraintestinal manifestation of inflammatory bowel disease (IBD). Although there are several causes of anemia in IBD, the two most frequent etiologies are iron deficiency anemia and anemia of chronic disease. Despite the high prevalence of anemia in IBD and its significant impact on patient's quality of life, this complication is still underdiagnosed and undertreated by providers. Active screening for anemia, structured assessment, comprehensive management, and multidisciplinary collaboration are needed in IBD patients. The cornerstone of anemia management depends on the underlying etiology along with normalization of inflammatory activity. Although, oral iron is effective for the treatment of mild iron deficiency-related anemia, intravenous iron formulations have a good safety profile and can be used as first-line therapy in patients with active IBD, severe anemia and previous intolerance prior to oral iron. After proper treatment of anemia, careful monitoring is necessary to prevent its recurrence. Herein, we discuss the etiology, screening, diagnosis, therapy selection, and follow-up for anemia in IBD.

Introducción: La anemia durante el embarazo se considera un problema de salud pública. La estrategia para su reducción incluye la administración de suplementos de hierro. Objetivo: Analizar la adherencia al suplemento de hierro en gestantes atendidas en los establecimientos de salud del Ministerio de Salud del Perú. Métodos: Se realizó un estudio transversal de vigilancia centinela de adherencia a sales de hierro en 1038 gestantes del Perú, atendidas en hospitales, centros de salud y puestos de salud, que recibieron suplementos de hierro. Se aplicó una entrevista estructurada para medir los factores a la adherencia y se determinó adherencia cuando el consumo de suplementos fuera > 75 por ciento. Se aplicó la estadística descriptiva y regresión logística. Resultados: El 42,3 por ciento de las gestantes fueron adherentes al suplemento de hierro. Se asociaron a la adherencia: tener educación universitaria (ORa 3,4; IC 95 por ciento 1,5:7,7), consumir alimentos ricos en hierro (ORa 1,6; IC 95 por ciento 1,1:2,5), no suspender el tratamiento por molestias (ORa: 2,8 IC 95 por ciento 1,7:4,6) y residir en una altitud entre 1001 m s. n. m. a 2000 m s. n. m. (ORa 4,0; IC 95 por ciento 1,6:10,1). Estos factores explican entre el 12 por ciento y el 16 por ciento de la adherencia, y fueron predictivos para identificar adherencia en gestantes con un área bajo la curva del 70,4 por ciento. Conclusiones: El tratamiento con sales de hierro en gestante es insuficiente, por lo que debe incluirse en su dieta, de manera integral y simultánea, el consumo de alimentos ricos en hierro y otros fortificados dirigidos básicamente a las embarazadas. Además, se deben trazar estrategias de salud que permitan identificar determinantes sociales, como el nivel educativo, que inciden en la adherencia al tratamiento, con vista a disminuir su influencia(AU)


Introduction: Anemia during pregnancy is considered a public health problem. The strategy for its reduction includes iron supplementation. Objective: Analyze adherence to iron supplements in pregnant women treated in health facilities of the Peruvian Ministry of Health. Methods: A cross-sectional sentinel surveillance study of adherence to iron salts was conducted in 1038 pregnant women from Peru, who were treated in hospitals, health centers and health posts, and who received iron supplements. A structured interview was applied to measure the factors to adherence, and adherence was determined when supplement consumption was > 75percent. Descriptive statistics and logistic regression were applied. Results: 42.3percent of the pregnant women were adherents to the iron supplement. Adherence was associated with: having a university education (ORa 3.4; CI 95percent 1.5:7.7), consumption of foods rich in iron (ORa 1.6; 95percent CI 1.1:2.5), do not discontinue the treatment due to discomfort (ORa: 2.8 CI 95percent 1.7:4.6) and living at an altitude between 1001 m.a.s.l to 2000 m.a.s.l (ORa 4.0; CI 95percent 1.6:10.1). These factors explain between 12percent and 16percent of the adherence rates, and were predictive to identify adherence in pregnant women with an area under the curve of 70.4percent. Conclusions: Treatment with iron salts in pregnant women is insufficient, so the consumption of foods rich in iron and other fortified foods basically aimed at pregnant women should be included in their diet, in a comprehensive and simultaneous way. In addition, health strategies must be drawn up to identify social determinants, such as educational level, that affect adherence to treatment, with a view to reduce their influence(AU)

Introduction: Anemias correspond to hematological disorders that can present in the oral cavity and face. Objective: To review the literature on the main types of anemic disorders and their orofacial manifestations, considering the aspects of interest to dentists. Methodology: This is a literature review, in which articles were selected in Portuguese and English, indexed in the Scielo, Medline/Pubmed and Lilacs databases with the descriptors: Anemia, Oral Manifestations, Jaw Abnormalities and their correspondents in Portuguese language. Literature review: Anemic disorders associated with orofacial signs and symptoms include mainly Iron-Deficiency, Megaloblastic, Fanconis, Sickle Cell, Thalassemia and Aplastic Anemia. The manifestations vary from burning and painful symptoms in the tongue, pallor of lips and mucosa, stomatitis, atrophic glossitis, angular cheilitis, susceptibility to candidiasis and peri-odontal disease. Also, dental changes, hyposalivation, malocclusion, osteomyelitis of the jaw, paraesthesia of the mental nerve and orofacial pain are included. Conclusion: These manifestations can be the first signs of the presence of anemia, which gives the dentist an important role in early diagnosis and proper management of dental treatment.


Introdução: As anemias correspondem a distúrbios hematológicos que podem apresentar manifestações na cavidade oral e face. Objetivo: Revisar a literatura acerca dos principais tipos de distúrbios anêmicos e suas manifestações orofaciais, considerando os aspectos de interesse aos cirurgiões-dentistas. Metodologia: Trata-se de uma revisão de literatura, em que foram selecionados artigos em português e inglês, indexados nas bases de dados do Scielo, Medline/Pubmed e no Lilacs, com os descritores: Anemia, Oral Manifestations, Jaw Abnormalities e seus correspondentes na língua portuguesa. Revisão de literatura: Os distúrbios anêmicos associados aos sinais e sintomas orofaciais incluem principalmente a Anemia Ferropriva, Megaloblástica, de Fanconi, Falciforme, Talassemia e Anemia Aplástica. As manifestações variam de ardência e sintomatologia dolorosa em língua, palidez de lábios e mucosa, estomatite, glossite atrófica, queilite angular, suscetibilidade a candidíase e doença periodontal. Ainda, englobam-se as alterações dentárias, hipossalivação, má oclusão, osteomielite da mandíbula, parestesia do nervo mental e dor orofacial. Conclusão: Essas alterações podem ser os primeiros sinais da presença da anemia, o que confere ao cirurgião-dentista um importante papel no seu diagnóstico precoce e condução adequada ao tratamento odontológico.

ABSTRACT

O HIV é o causador da AIDS, doença que representa um dos grandes problemas de saúde pública em todo o mundo. Apesar de avanços na terapia antirretroviral no sentido de aumentar a expectativa de vida dos indivíduos infectados, as alterações hematológicas, como a anemia, acompanham o curso clínico da doença. Essa condição, normalmente multifatorial, pode estar presente em qualquer fase da doença e afeta diretamente o prognóstico e a qualidade de vida do indivíduo infectado. Este artigo apresenta informações do surgimento do processo anêmico com base nas principais causas encontradas na literatura.


The human immunodeficiency virus is the cause of the acquired immunodeficiency syndrome, a disease that represents one of the major public health problems worldwide. In spite of advances in antiretroviral therapy that increase patients' life expectancy, hematological changes, such as anemia, follow the clinical course of the disease. This commonly multifactorial condition can be found in any phase of the disease and directly affects the prognosis and quality of life of patients. This article presents information on the emergence of the anemic process based on the main causes found in the literature.

ABSTRACT

Este trabajo tiene como objetivo revisar las contribuciones de la biotecnología, en relación con el tratamiento, diagnóstico y la monitorización de la enfermedad renal crónica (ERC) y sus comorbilidades más frecuentes, especialmente la anemia. En relación con los tratamientos, enfocamos el desarrollo de productos biofarmacéuticos como los agentes estimulantes de la eritropoyesis (ESA), que fueron los primeros biofármacos utilizados para el tratamiento de la anemia asociada a la ERC; analizamos sus características y utilización actual después de varios años de experiencia clínica, así como también otras alternativas en desarrollo. Revisamos distintos tipos de bioterapias, la utilización de las células estromales mesenquimales de médula ósea (MSC) y tratamientos alternativos con modificaciones dietarias, que se basan en la asociación entre la microbiota intestinal de los pacientes renales crónicos y sus condiciones fisiopatológicas. Finalmente, en relación con el diagnóstico y monitorización, nos referimos al estudio y validación de biomarcadores diagnósticos, predictivos y terapéuticos que han permitido optimizar los resultados clínicos en este tipo de pacientes. The aim of this work is to review the contributions of biotechnology, in relation to the treatment, diagnosis and monitoring of chronic kidney disease (CKD) and its most frequent comorbidities, especially anemia. Regarding the treatment, we focus on the development of biopharmaceutical products such as erythropoiesis stimulating agents (ESA), which were the first biopharmaceuticals used to treat anemia associated with chronic kidney disease (CKD). We analyzed their characteristics and their current use after several years of clinical experience, as well as other alternatives in development. We also review different types of biotherapies, the use of bone marrow mesenchymal stromal cells (MSC) and alternative treatments with dietary modifications, which are based on the association between the intestinal microbiota of chronic kidney patients and their pathophysiological conditions. Finally, in relation to diagnosis and monitoring, we refer to the study and validation of diagnostic, predictive and therapeutic biomarkers that have made clinical results possible to be optimized in this type of patient. Introducción. La anemia en los primeros años de vida produce graves consecuencias psicomotoras, sociales y económicas. El objetivo fue determinar su prevalencia, causas y factores de riesgo y preventivos en lactantes de la ciudad de Necochea.Población y métodos. Estudio observacional, descriptivo y transversal. Se evaluaron lactantes sanos de 6 a 12 meses, que concurrieron espontáneamente por control de salud al sistema público y/o privado del distrito de Necochea durante el año 2017. Se solicitó consentimiento informado; se realizó una encuesta social y nutricional, evaluación del aporte de hierro de la dieta y medicamentoso, examen físico y pruebas de laboratorio.Resultados. Se incluyeron 239 participantes; el 50,6 % presentaban anemia y el 47,3 %, ferropenia. La media y mediana de hemoglobina coincidieron en 10,9 g/dl, (media de referencia 12,5 mg/dl). De los anémicos, el 54,4 % presentaba ferropenia; el 61,7 % no recibía aporte adecuado de hierro; el 44,3 % no había recibido hierro suplementario el día previo. De este porcentaje, el 24,5 % no había recibido indicación médica y, en el 9,1 %, no había suplemento en el centro de atención primaria de salud u hospital. El aporte adecuado de hierro resultó un factor protector para ferropenia (riesgo relativo ­RR­ 0,78 [intervalo de confianza ­IC­ del 95 %: 0,6-0,9]), y no fue así para la aparición de anemia (RR 1,08; [IC95%: 0,8-1,3]).Conclusiones. En la ciudad de Necochea, anemia y ferropenia son entidades con elevada prevalencia; el aporte de hierro dietario es pobre y el suplemento está subindicado.


Introduction. Anemia in the first years of life leads to severe psychomotor, social, and financial effects. The objective of this study was to determine its prevalence, causes, and risk and preventive factors among infants in the city of Necochea.Population and methods. Observational, descriptive, and cross-sectional study. Healthy infants aged 6-12 months who attended a spontaneous health checkup in a public and/or private facility in the district of Necochea during 2017 were assessed. An informed consent was obtained; a social and nutrition survey was administered; dietary and medicinal iron intake was assessed; and a physical examination and lab tests were done.Results. A total of 239 participants were included; 50.6 % had anemia and 47.3 %, iron deficiency. Mean and median hemoglobin levels were both 10.9 g/dL, (reference mean: 12.5 mg/dL). Among anemia patients, 54.4 % had iron deficiency; iron intake was inadequate in 61.7 %; and 44.3 % had not received iron supplementation the previous day. Among these, 24.5 % had not received a medical indication for it, and 9.1 % did not find iron supplementation available at their primary health care center or hospital. An adequate iron intake was a protective factor against iron deficiency (relative risk [RR]: 0.78 [95 % confidence interval {CI}: 0.6-0.9]), but not against the development of anemia (RR: 1.08; [95 % CI: 0.8-1.3]).Conclusions. In the city of Necochea, anemia and iron deficiency are highly prevalent conditions; dietary iron intake is insufficient and supplementation is under-prescribed.

Objetivo: Comparar características sociodemográficas, comportamentais, condições de saúde e parâmetros laboratoriais entre idosos anêmicos e não anêmicos. Métodos: Estudo epidemiológico transversal realizado com 257 idosos. Para realizar as comparações utilizou-se o teste t para amostras independentes e o teste não paramétrico U de Mann-Whitney. Resultados: Encontrou-se maior proporção de anemia em idosos com idade acima de 80 anos (p=0,026), e maior proporção de dependência para atividades básicas (p=0,018) e instrumentais (p=0,010) da vida diária em idosos com anemia. Identificou-se diferença na distribuição da dosagem dos parâmetros laboratoriais, sendo as concentrações médias de ureia e creatinina maiores nos idosos anêmicos, e esses tiveram menores concentrações de glicemia, albumina, ALT/TGP, cálcio sérico e T3 total. Conclusão: A anemia não está relacionada exclusivamente ao processo de envelhecimento, mas sim resultado de múltiplos fatores como as condições sociodemográficas, de saúde, alimentares e de vida dos idosos


Objective: To compare sociodemographic, behavioral characteristics, health conditions and laboratory parameters among anemic and non - anemic elderly. Methods: Cross-sectional epidemiological study with 257 elderly subjects. To perform comparisons, the t-test for independent samples and the non-parametric Mann-Whitney U test were used. Results: There was a higher proportion of anemia in the elderly over 80 years (p = 0.026), and a higher proportion of dependence on basic activities (p = 0.018) and instrumental (p = 0.010) daily activities in elderly patients with anemia. Differences were observed in the distribution of laboratory parameters, with mean values of urea and creatinine being higher in anemic elderly, with lower concentrations of glycemia, albumin, ALT / TGP, serum calcium and total T3. Conclusion: Anemia is not exclusively related to the aging process, but is a result of multiple factors such as sociodemographic, health, food and life conditions of the elderly


Objetivo: Comparar características sociodemográficas, comportamentales, condiciones de salud y parámetros de laboratorio entre ancianos anémicos y no anémicos. Metodo: Estudio epidemiológico transversal realizado con 257 ancianos. Para realizar las comparaciones se utilizó la prueba t para muestras independientes y la prueba no paramétrica U de Mann-Whitney. Resultados: Se encontró mayor proporción de anemia en ancianos con edad superior a 80 años (p = 0,026), y mayor proporción de dependencia para actividades básicas (p = 0,018) e instrumentales (p = 0,010) de la vida diaria en ancianos con anemia. Se identificó diferencia en la distribución de la dosificación de los parámetros de laboratorio, siendo las concentraciones medias de urea y creatinina mayores en los ancianos anêmicos. Conclusión: La anemia no está relacionada exclusivamente con el proceso de envejecimiento, sino el resultado de múltiples factores como las condiciones sociodemográficas, de salud, alimentos y de vida de los ancianos

Os receptores de transplante renal são mais suscetíveis a infecções, entre elas o parvovírus B19, que pode ser transmitido por via respiratória, adquirido por meio do enxerto ou por reativação de infecção latente. A anemia normocítica normocrômica, com diminuição dos reticulócitos e resistência ao tratamento com eritropoietina, é a principal forma de apresentação da infecção por parvovírus B19 em transplante renal. O diagnóstico requer alto índice de suspeição clínica e realização de testes diagnósticos selecionados. Tratamento com imunoglobulina e suspensão dos imunossupressores durante a infecção mostraram-se eficazes. Os autores relatam sua experiência com cinco casos de infecção por parvovírus B19 em receptores de transplante renal de um hospital universitário. Os aspectos clínicos, diagnósticos e terapêuticos são revistos.


Kidney transplant recipients are more susceptible to infections, including by parvovirus B19, spread through the respiratory tract, acquired through the graft or reactivation of latent infection. Normocytic normochromic anemia, with decreased reticulocytes and resistance to erythropoietin treatment, is the most common presentation of Parvovirus B19 infection in renal transplant. Diagnosis requires a higher clinical suspicion and the performance of selected diagnostic tests. Treatment with immunoglobulin and suspension of immunosuppressive therapy during the infection may be effective. The authors report five cases of PB19 infection in kidney transplant patients at a hospital. The clinical, diagnostic, and treatment features are reviewed.

Bachkgrond: Anemia is a very important nutritional disorder in the world, it has multiple etiologies and pregnancy is associated with a deficit in the intake of iron and physiological changes of pregnancy. Aim of the study: To assess the prevalence of anemia in pregnant Iraqi women. Patients and method: A cross-sectional study carried at Al-Elwyia maternity teaching hospital in the period from the 1st of Jan. 2018 to the end of October 2018, in which 1000 pregnant women were enrolled in the study and divided into 2 groups: case group (n=652) and control group (n=348) according to the level of Hb. Results: The mean age of the case group was (27.8±7.1) years old and (26.1±2.3) years for the control group. The anemia was found in (6521000) patients and Iron deficiency anemia is the main type of anemia (80.2%) while (19.8%) was another type of anemia. Poor diet (65%) is the common cause of anemia were found commonly in the 3rd trimester (50.2%). Conclusion: The prevalence of anemia was 65.2% in the current study (AU)

ABSTRACT

Background: Anemia is considered by Worl Health Organization (WHO) as a global health problem. It can be affected by the body mass index of an individual since increasing weight can result in low-grade systemic inflammation and elevation of hepcidin which results in seguestratiion of iron inside a variety of cells including macrophages, hepatocytes and enteroytes and this would lead to the development of what is known as anemia of inflammation. In this study, we aimed to evaluate the effects of BI on complete blood count parameters. Patients and methods: In this crosssectional, retrospective study: the data of 200 overweight and obese male patients aged (18-60 years) were collected. They were grouped according to BMI into everweight and obese: different CBC parameters were noted. Inter group comparison was applied regarding different blood parameters and the relations eith BMI were calculated. Results: The prevalence of anemia was 16%. An extremely significant difference was found in mean white blood cells count in relation to different BMI gropups (p=0.001). A positive linear correlation was found between BMI with RDW, WBS, and platelets. Conclusions: There were effects of increasing BMI on WBC, RDW and platelets (AU)

ABSTRACT

A anemia é muito prevalente em idosos; todavia, a eliptocitose hereditária, uma anemia hereditária caracterizada pela presença de eritrócitos em forma elíptica no sangue periférico; raramente causa anemia sintomática em pacientes idosos. A eliptocitose esferocítica é uma anemia hereditária com caráter autossômico dominante. A associação entre essas duas eritroenzimopatias hereditárias é rara e seu curso varia com alterações da forma eritrocitária, observando-se a presença simultânea de eliptocitose e esferocitose no sangue periférico. A hemólise pode variar de média a moderada intensidade. Relatamos o caso de uma paciente adulta idosa com eliptocitose esferocítica revelada após 26 anos, sendo realizada a esplenectomia para resolução do quadro de hemólise.


Anemia is very prevalent in the elderly; however, hereditary elliptocytosis, an inherited anemia characterized by the presence of elliptical erythrocytes in the peripheral blood; rarely causes symptomatic anemia in elderly patients. Spherocytic elliptocytosis is an inherited anemia with an autosomal dominant character. The association between these two hereditary erythroenzyopathies is rare and its course varies with alterations of the erythrocyte form, observing the simultaneous presence of elliptocytosis and spherocytosis in the peripheral blood. Hemolysis can range from medium to moderate intensity. We report the case of a patient with spherocytic elliptocytosis revealed after 26 years, and splenectomy was performed to resolve the hemolysis.

ABSTRACT

INTRODUCTION: Perioperative anemia in patients with cancer is frequent and is associated with potential adverse outcomes. In association studies it was established that anemia changes the prognosis in some cases, being associated with a possible increase in the risk of disease progression and death. On the other hand, it has been proposed that immunomodulation secondary to transfusions favors metastasis and the disease`s recurrence. OBJECTIVE: To describe the current evidence on anemia, transfusions and other perioperative strategies for the management of patients with anemia and cancer taken to surgery. MATERIALS AND METHODS: A non-systematic review of the literature that included articles on the perioperative management of anemic patients candidates for oncological surgery was performed. The main findings are described and the evidence is presented in favor of and against some strategies (other than red blood cell transfusion) to correct anemia. CONCLUSION: Optimization of the erythrocyte mass before surgery and the use of blood-sparing strategies should be considered in a comprehensive perioperative management.


INTRODUCCIÓN: La anemia perioperatoria en el paciente con cáncer es frecuente y se asocia a potenciales desenlaces adversos. En estudios de asociación se estableció que la anemia cambia el pronóstico en algunos casos, asociándose a un posible incremento del riesgo de progresión de la enfermedad y muerte. Por otra parte, se ha propuesto que la inmunomodulación secundaria a las transfusiones, favorece las metástasis y la recidiva de la enfermedad. OBJETIVOS: Describir la evidencia actual sobre la anemia, las transfusiones y otras estrategias perioperatorias para el manejo de los pacientes con anemia y cáncer llevados a cirugía. MATERIALES Y MÉTODOS: Se realizó una revisión no sistemática de la literatura que incluyó artículos sobre el manejo perioperatorio de pacientes anémicos candidatos a cirugía oncológica. Se describen los principales hallazgos y se presenta la evidencia a favor y en contra de algunas estrategias (diferentes a la transfusión de glóbulos rojos) para corregir la anemia. CONCLUSIÓN: La optimización de la masa eritrocitaria antes de la cirugía y el uso de estrategias ahorradoras de sangre deben ser consideradas en un manejo perioperatorio integral.

Background: Anaemia in pregnancy is associated with adverse obstetric outcomes. When detected early in pregnancy, it can be treated; however, information on its prevalence and associated factors is limited in rural Ghana.Aim: The aim of this study was to determine the prevalence and maternal factors associated with anaemia in pregnancy at first antenatal care (ANC) visits.Setting: The study was conducted in the Navrongo War Memorial Hospital, a secondary referral facility in the Kassena-Nankana district in rural northern Ghana.Methods: A retrospective analysis of antenatal clinic records of pregnant women collected from January to December 2014. All pregnant women initiating antenatal clinic, who had initial haemoglobin (Hb) levels measured, were included in the study. Logistic regression analyses were carried out to determine factors associated with anaemia at the initiation of ANC.Results: We analysed data from 506 women with median Hb of 11.1 g/dL (IQR 7.31­13.8). The median gestational age at booking was 14 weeks (5­36 weeks). The prevalence of anaemia was 42.7%, with 95% confidence interval (CI) [38.4­47.1], and was high among teenage mothers (52% [34.9­67.8]), mothers who booked in the third trimester (55% [33.6­74.7]) and grand multiparous women (58% [30.7­81.6]). Factors associated with anaemia included grand multiparity (odds ratio [OR] = 1.94 with 95% CI [1.58­2.46]), booking during the third trimester (OR = 2.06 [1.78­2.21]) and mother who were underweight compared to those with normal weight (OR = 3.17 [1.19­8.32]).Conclusion: Burden of anaemia in pregnancy is still high in rural northern Ghana. We advocate further strengthening of the primary health care system to improve early access to ANC deliver
